With the junk I have on hand, rear wheel hub bodies seem about 6mm narrower than front.

The fix that occurs is to cut a proper keyway in the axle and hub bushing. There may be space for a disk brake too.

It could just as well failed by gouging a groove in the aluminium hub. This is terrible mechanical design. I hope it’s only an early example

One wheel spins freely because the D-hole in the washer has worn itself round. Damaging the thread while doing so.

Axle is smooth. Small flat on the retaining thread keys to a lugged washer. Single lug transfers all force to the wheel hub.
